The focus was not only on the game's realism—from caring for babies to driving cars—but also heavily centered on the depiction of adult content. Lee navigated the topic with care, stating, "Our answer is: sort of. There is a feature in the game where it's clear what's happening; the male Zoi and the female Zoi are together, likely in bed, with the intention of creating babies. The visual representation, however, leaves much to the imagination. What you see is not explicitly what's happening; you're meant to infer the activity. Is that clear enough for everyone? It's there. Sort of. Maybe not to the level that everyone would have been expecting."
