Before you start reeling in the aquatic bounty, you'll need to accept a quest. Find Simon the Fisherman (the white-haired NPC near the Barracuda's swimming area). He'll task you with catching 5 "fish"—we'll explain the quotes later. In return, he'll reward you with a handy Tackle Box.

To catch those fish, you'll need a fishing rod and bait. Conveniently, Simon sells both. Buy a Wood Fishing Rod and some Worms. We recommend at least 5 worms, but 10 provides a safety net. Important note: you don't equip the bait. Simply having the Worms in your inventory is key. Each successful catch (or unsuccessful one, as you'll see) consumes a Worm.
Here's the crucial detail: you need *at least* 5 worms in your inventory to catch anything. We initially tried with fewer and had no luck. Keep your Worms readily accessible—placing them in your hotbar is a good strategy.
How to Catch Fish in Rune Slayer

Select your Wood Fishing Rod (you don't equip it, just have it selected in your hotbar). Hold down M1 to cast your line into the water (the pier near Simon is perfect). Watch your bobber. When it ripples (once or twice), click M1 again to reel in your catch.
Important: This isn't foolproof. Often you'll catch nothing, or you'll catch junk. The good news? Simon counts junk as a catch (hence the "fish" quotes earlier). Cast, wait for ripples, click M1—repeat until you've got 5 catches.
